چکیده

Methods In this retrospective case note study we looked at the outcome of each extubation between 1st October 2013 and 30th September 2014. We included all patients >18 years of age invasively ventilated for >48 hours. We excluded patients proceeding directly to tracheostomy or extubated for withdrawal of care. Failed extubation was defined as the need for reintubation within 5 days of planned extubation. Data was analysed in two groups; ́success ́ and ́failure ́ of extubation.

Planned Versus Unplanned Extubation in Opioid Overdose Patients: Does it have any effect on the prognosis? A cohort study

Background: Tracheal intubation is a life-saving measure in patients poisoned acutely with opioid, and when naloxone treatment is inadequate. This study determined the risk factors for early unplanned extubation in these patients and evaluated the effects on the clinical outcomes. Methods: At a poisoning center in Tehran, Iran, 165 opioid overdose patients who were admitted to the intensive ca...

Clinical review: Post-extubation laryngeal edema and extubation failure in critically ill adult patients

Laryngeal edema is a frequent complication of intubation. It often presents shortly after extubation as post-extubation stridor and results from damage to the mucosa of the larynx. Mucosal damage is caused by pressure and ischemia resulting in an inflammatory response. Laryngeal edema may compromise the airway necessitating reintubation. Several studies show that a positive cuff leak test combi...

Easily identified at-risk patients for extubation failure may benefit from noninvasive ventilation: a prospective before-after study

BACKGROUND While studies have suggested that prophylactic noninvasive ventilation (NIV) could prevent post-extubation respiratory failure in the intensive care unit, they appear inconsistent with regard to reintubation. We assessed the impact of a prophylactic NIV protocol on reintubation in a large population of at-risk patients. METHODS Prospective before-after study performed in the medica...

Extubation failure: magnitude of the problem, impact on outcomes, and prevention.

Extubation failure, defined as the need for reinstitution of ventilatory support within 24 to 72 hours of planned endotracheal tube removal, occurs in 2 to 25% of extubated patients. The pathophysiologic causes of extubation failure include an imbalance between respiratory muscle capacity and work of breathing, upper airway obstruction, excess respiratory secretions, inadequate cough, encephalo...
